Transcriptional enhancer sequences have been shown to play a pivotal role in the regulation of some highly expressed genes. First described in eukaryotic viruses, the discovery of enhancers has augmented the previously defined control‐sequence motifs to give a more complete understanding of eukaryotic transcriptional regulatory mechanisms. Some properties of enhancers that distinguish them from other regulatory sequences include their ability to function in a position‐ and orientation‐independent manner. Furthermore, the observation that some enhancers and transcriptional promoters exhibit tissue specificity in (...) their activity has generated considerable interest. Enhancer activation is thought to involve sequence‐specific DNA‐binding proteins but the process by which this leads to an increase in the initiation of transcription remains obscure. Although viral enhancers are more fully characterized, several cellular enhancers have recently been identified. One such enhancer sequence has been detected in a 3′ flanking region of a highly expressed human placental lactogen gene. This enhancer can increase the transcriptional rate of an adjacent gene by at least 50‐fold. (shrink)

Background: Ethical issues related to comparative effectiveness research, or research that compares existing standards of care, have recently received considerable attention. In this paper we focus on how Ethics Review Committees should evaluate the risks of comparative effectiveness research. Main text: We discuss what has been a prominent focus in the debate about comparative effectiveness research, namely that it is justified when “nothing is known” about the comparative effectiveness of the available alternatives. We argue that this focus may be misleading. (...) Rather, we should focus on the fact that some experts believe that the evidence points in favor of one intervention, whereas other experts believe that the evidence favors the alternative. We will then introduce a case that illustrates this point, and based on that, discuss how ERCs should deal with such cases of expert disagreement. Conclusion: We argue that ERCs have a duty to assess the range of expert opinions and based on that assessment arrive at a risk judgment about the study under consideration. We also argue that assessment of expert disagreement is important for the assignment of risk level to a clinical trial: what is the basis for expert opinions, how strong is the evidence appealed to by various experts, and how can clinical trial monitoring affect the possible increased risk of clinical trial participation. Purpose/methods: This study investigated the relationship between ethics education and training, and the use and usefulness of ethics resources, confidence in moral decisions, and moral action/activism through a survey of practicing nurses and social workers from four United States (US) census regions. Findings: The sample (n = 1215) was primarily Caucasian (83%), female (85%), well educated (57% with a master's degree). no ethics education at all was reported by 14% of study participants (8% of social workers had no ethics education, (...) versus 23% of nurses), and only 57% of participants had ethics education in their professional educational program. Those with both professional ethics education and in-service or continuing education were more confident in their moral judgments and more likely to use ethics resources and to take moral action. Social workers had more overall education, more ethics education, and higher confidence and moral action scores, and were more likely to use ethics resources than nurses. Conclusion: Ethics education has a significant positive influence on moral confidence, moral action, and use of ethics resources by nurses and social workers. (shrink)

In spring 2020, COVID-19 and the ensuing social distancing and stay-at-home orders instigated abrupt changes to employment and educational infrastructure, leading to uncertainty, concern, and stress among United States college students. The media consumption patterns of this and other social groups across the globe were affected, with early evidence suggesting viewers were seeking both pandemic-themed media and reassuring, familiar content. A general increase in media consumption, and increased consumption of specific types of content, may have been due to media use (...) for coping strategies. This paper examines the relationship between the stress and anxiety of university students and their strategic use of media for coping during initial social distancing periods in March-April 2020 using data from a cross-sectional survey. We examine links between specific types of media use with psychological well-being concepts, and examine the moderating roles of traits (hope, optimism, and resilience) as buffers against negative relationships between stress and anxiety and psychological well-being. Our findings indicate that stress was linked to more hedonic and less eudaimonic media use, as well as more avoidant and escapist media-based coping. Anxiety, on the other hand, was linked to more media use in general, specifically more eudaimonic media use and a full range of media-based coping strategies. In turn, escapist media was linked to negative affect, while reframing media and eudaimonic media were linked to positive affect. Avoidant coping was tied to poorer mental health, and humor coping was tied to better mental health. Hedonic and need-satisfying media use were linked to more flourishing. Hope, optimism, and resilience were all predictive of media use, with the latter two traits moderating responses to stress and anxiety. The findings give a nuanced portrait of college students’ media use during a pandemic-induced shutdown, showing that media use is closely intertwined with well-being in both adaptive and maladaptive patterns. Psychiatry is rapidly adopting digital phenotyping and artificial intelligence/machine learning tools to study mental illness based on tracking participants’ locations, online activity, phone and text message usage, heart rate, sleep, physical activity, and more. Existing ethical frameworks for return of individual research results (IRRs) are inadequate to guide researchers for when, if, and how to return this unprecedented number of potentially sensitive results about each participant’s real-world behavior. To address this gap, we convened an interdisciplinary expert working group, supported by (...) a National Institute of Mental Health grant. Building on established guidelines and the emerging norm of returning results in participant-centered research, we present a novel framework specific to the ethical, legal, and social implications of returning IRRs in digital phenotyping research. Our framework offers researchers, clinicians, and Institutional Review Boards (IRBs) urgently needed guidance, and the principles developed here in the context of psychiatry will be readily adaptable to other therapeutic areas. Over the course of human life, health care decision‐making is often interdependent. In this article, we use “interdependence” to refer to patients’ engagement of nonclinicians—for example, family members or trusted friends—to reach health care decisions. Interdependence, we suggest, is common for patients in all stages of life, from early childhood to late adulthood. This view contrasts with the common bioethical assumption that medical decisions are either wholly independent or dependent and that independence or dependence is tightly coupled with a person's (...) decision‐making capacity. In this article, we array various approaches to decision‐making along a continuum of interdependence. An appreciation of this continuum can empower patients and elucidate ethical challenges that arise when people transition between different kinds of interdependence across the life span. (shrink)

The inclusion of stakeholders and knowledge systems is increasingly valued in research to address complex socio-ecological challenges around the world. Often these projects take place in cross-cultural setting where external researchers risk perpetuating historically extractive research models that not only harm local communities but damage the validity of research projects. Responsible community engagement is increasingly recognized as a practice that can improve researcher-community relationships and research quality by incorporating principles of ethics, reciprocity, and power sharing. In partnership with local community (...) leaders, researchers from a U.S. university coded 76 research articles for indicators of responsible community engagement conducted in the Samburu, Kenya community since 2000. Findings from this study suggest that most of the research in Samburu has followed extractive models. Of the 76 articles reviewed many failed to acknowledge ethical protocols, did not address relevance of the study to the local community, and did not report any indication of outreach or community change as a result of the research. While a portion of articles showed evidence of community involvement in the studies, the involvement was primarily limited to secondary roles with little to no shared decision-making power over the research process. We discuss methodological considerations for future research and steps that must be taken in order to shift the norm and practice of academic research in sites such as Samburu, Kenya. (shrink)

Background: During the COVID-19 pandemic, frontline workers faced a series of challenges balancing family and work responsibilities. These challenges included making decisions about how to reduce COVID-19 exposure to their families while still carrying out their employment duties and caring for their children. We sought to understand how frontline workers made these decisions and how these decisions impacted their experiences.Methods: Between October 2020 and May 2021, we conducted 61 semi-structured interviews in English or Spanish, with individuals who continued to work (...) outside of the home during the pandemic and had children living at home. Interviews were recorded, transcribed verbatim, and analyzed using abductive methods.Results: Frontline workers experienced moral distress, the inability to act in accordance with their values and obligations because of internal or external constraints. Their moral distress was a result of the tensions they felt as workers and parents, which sometimes led them to feel like they had to compromise on either or both responsibilities. Individuals felt morally conflicted because 1) their COVID-19 work exposures presented risk that often jeopardized their family’s health; 2) their work hours often conflicted with their increased childcare responsibilities; and 3) they felt a duty to their colleagues, patients/customers, and communities to continue to show-up to work.Conclusions: Our findings point to a need to expand the concept of moral distress to include the perspectives of frontline workers outside of the healthcare professions and the fraught decisions that workers make outside of work that may impact their moral distress. Expanding the concept of moral distress also allows for a justice-based framing that can focus attention on the disparities inherent in much frontline work and can justify programmatic recommendations, like increasing paid childcare opportunities, to alleviate moral distress. Pourquoi s'exile-t-on? Pour échapper à l'injustice, reconquérir une liberté menacée, fuir les violences, les persécutions, la mort, la misère ou s'arracher à la résignation. On part quand rien ne fait plus écran au risque d'anéantissement, que l'espérance devient lettre morte, alors même que la vie n'a pas été accomplie. Les épreuves qui conduisent aujourd'hui à l'exil ont leurs origines dans les déchirures du nouvel ordre mondial : la guerre économique sans merci des états de la planète ; l'incapacité à maîtriser (...) la réalité du marché financier ; le chômage de masse ; l'exclusion des citoyens sans abri de toute participation à la vie démocratique des états ; l'aggravation de la dette qui affame et réduit au désespoir une grande part de l'humanité ; les guerres interethniques qui se multiplient, guidées par le fanatisme et les fantasmes de la communauté absolue ; les effets destructeurs des dictatures ; les pathologies de l'identité collective fondées sur l'idéalisation de la haine ; la violence naturalisée réduite à une simple gestion ; la cruauté ; le nettoyage ethnique, etc. Pour ces raisons, il est temps de faire de l'exil une catégorie politique de portée universelle et signifiante pour la modernité. Prendre en compte cette exigence permet de repenser les termes de la politique afin de sortir de la passion identitaire et de poser la seule question qui vaille : est-ce qu'on peut faire quelque chose et sous quelle forme ou est-ce qu'on ne peut rien? L'objectif est d'esquisser une éthicosmopolitique qui se présente comme une politique de la condition humaine, un pari sur la capacité de chacun de répondre sans exception à la vulnérabilité d'autrui, un tout-autre-être-au-monde, une forme de vie qui nous lie les uns aux autres. L'enjeu est considérable. Penser politiquement l'exil, c'est chercher à comprendre ce que veut dire être-ensemble, être au monde, être sujet ; c'est ouvrir le monde à la totalité des possibilités qu'il contient. (shrink)

L’objectif de cet article est d’interroger la transition Kant-Fichte sur le double plan de la systématicité et du primat de la raison pratique. S’il est admis que Kant voyait dans l’exercice critique l’occasion de renouveler, plutôt que de supprimer, la possibilité d’une métaphysique positive, il est aujourd’hui convenu de rapporter son ébauche de «système» au seul héritage de laSchulmetaphysik. A contrario, on cherche ici à prendre à la lettre l’exigence kantienne d’un système qui attend du «pratique» une nouvelle et absolue (...) «fondation», plutôt qu’il ne devrait être complété en l’état. La perspective fichtéenne impose alors son fil rouge.In this article, I examine two aspects of the transition from Kant to Fichte: systematicity and the primacy of practical reason. It is commonly agreed that Kant saw the critical exercise as an occasion to renew, rather than supress, the possibility of a positive metaphysics; however, most scholars now consider the “system” he sketched to be the sole heritage ofSchulmetaphysik. However, I take literally Kant’s demand for a system that expects from the “practical” a new and absolute “foundation,” rather than a system that must be completed. L’objet de cet article consiste à interroger le traitement que subit le concept de « représentation » dans cette phase cruciale de la modernité que constituent l’idéalisme fichtéen et sa réception romantique, en particulier dans le travail de Novalis. La crise que subit la représentation chez Fichte va de pair avec la promotion d’un type de réflexivité inédit, dont Novalis hérite tout en en décalant, par un jeu de miroir déformant, les conséquences méthodologiques voulues par son maître. Le biais proposé (...) consiste en une lecture suivie de Die Lehrlinge zu Saïs, le « roman de la nature » de Novalis. On verra comment, investiguant l’espace, le Disciple s’éduque à la représentation, c’est-à-dire à l’image et à son intransitivité essentielle, à laquelle le langage lui-même le conduit, sans que la réflexivité de l’apprentissage ne soit sacrifiée un seul instant. Le but est de montrer de quelle manière la philosophie transcendantale, pour Novalis, loin de se muer en un irrationalisme complaisant, doit, pour faire valoir sa « scientificité », thématiser pour lui-même l’acte singulier de son invention. Peut-être, d'ailleurs, n'est-ce qu'en tant qu'elle est défictionnalisée que la fiction peut promouvoir ou publier une morale. Mais il est, pourtant, des morales qui contiennent une part constitutive de fiction, et qui, en quelque sorte, (...) appellent la fiction : ainsi de la morale intuitive de Victor Hugo, ou de la morale de la liberté de Sartre. La Fontaine, La Rochefoucauld, Vauvenargues, Hugo, Tolstoï, Sartre : à première vue, les auteurs convoqués ne sont pas tous faits pour s'entendre. C'est bien pour cela qu'ils sont ici réunis. In this paper, I will try to analyze St. Augustine’s and Dante’s views towards history by showing how their visions can be articulated into particular models of history, i.e. a particular schema that describes the unfolding of history with its specific focal points, and to what extent this model differs from one author to another. If in Augustine’s case, by exploring his division of sacred history in component parts, that can be found throughout his work, I will argue (...) for a Christocentric model, in Dante’s case, by analyzing his work on political philosophy, De monarchia, I will argue for a teleological model of history. Plusieurs générations chrétiennes ont été convaincues que le christianisme était la seule vraie religion du salut parce qu’il a eu l’audace de dire que Dieu est venu dans l’histoire et y a posé des actes de salut. Et cela a influencé un langage de l’exclusivité. Cependant, la proximité physique des grandes religions du monde et la connaissance que nous en avons aujourd’hui, ne nous obligent-elles pas d’abandonner ce discours exclusiviste? Et la théologie chrétienne des religions ne devrait-elle pas, à partir (...) des “relativités immanentes” au christianisme, faire place à un langage de relativité?
